Client Services Agreement
Please read carefully, then complete and sign below before your first appointment.
1. Nature of Services
Sage Consulting and Wellness LLC provides health and wellness consulting services to support midlife women in organizing their health information, setting wellness goals, and navigating the healthcare system. My approach is grounded in current research on women's health, chronic illness, and self-advocacy, and draws on that research to help you better understand your own health picture and communicate it effectively to your care team.
Services are educational and coaching-based in nature. These services are not medical care, mental health therapy, or clinical treatment. I do not diagnose, treat, or prescribe.
When it's helpful, I may refer you to licensed, credentialed professionals — such as physicians, therapists, or specialists — for care that falls outside the scope of this work. Any referrals I make are to providers I've vetted and trust.
2. Geographic Scope
Services are available to residents of Illinois. If you are temporarily traveling outside of Illinois and wish to meet virtually, this is permitted provided your primary residence is in Illinois.
3. Who I Work With
I work with adults 18 years of age and older.
4. Services and Fees
Services are provided virtually. Current service offerings and fees are as follows:
Packages:
- Single Intake Appointment — $650 / 90 minutes
- Foundation Package — $4,000 / 3 months (includes 90-minute intake + 6 appointments)
- Empowered Advocate Package — $6,500 / 6 months (includes 90-minute intake + 12 appointments)
- VIP Transformation Program — $10,500 / 12 months (includes 90-minute intake + 24 appointments)
Stand-Alone Services:
- Power Hour Consultation — $400 / 60 minutes
- Let's Get Organized — $600 / 90 minutes
- Medical Appointment Prep — $200 / 30 minutes
- Advocacy Training — $200 / 30 minutes or $400 / 60 minutes
- Accountability Check — $100 / 20 minutes
If you complete a Single Intake Appointment and book a package within 14 days, the $650 intake fee will be applied toward your package total.
Fees are subject to change. Current fees are reflected on the website at mysageconsulting.com.
5. Payment
Payment is collected securely through Stripe at the time of scheduling.
For package services, two payment options are available:
- Pay in full: Full payment is due at the time of scheduling. A 10% discount is applied to the total package fee.
- Split payment: 50% of the package fee is due at the time of scheduling. The remaining 50% is due at the start of appointment 4 (Foundation), appointment 7 (Empowered Advocate), or appointment 13 (VIP Transformation Program).
A card on file is required for all package services to support the cancellation and no-show policies outlined in Section 6.
For the Single Intake Appointment and all stand-alone services, a 50% deposit is due at the time of scheduling, with the remaining balance charged to the card on file.
6. Cancellation Policy
I understand that life happens. I ask that you provide at least 48 business hours advance notice if you need to cancel or reschedule your appointment.
Late cancellations (less than 48 business hours notice) will incur a fee charged to the card on file.
- For single intake appointments and stand-alone services, the fee is 50% of the scheduled appointment fee — the deposit collected at scheduling serves this purpose and will not be refunded.
- For package appointments, the late cancellation fee is a flat $250, charged to the card on file. Your appointment will be rescheduled and does not count against your remaining package sessions.
No-shows without advance notice will be charged 100% of the scheduled appointment fee, charged to the card on file.
- For single intake appointments and stand-alone services, this means the deposit is forfeited and the remaining balance is charged.
- For package appointments, the missed session is forfeited and counts as used toward your package. If you would like to schedule a replacement session, it will be booked and billed separately as a Power Hour appointment at the standard rate.
If you experience a true emergency, please reach out as soon as possible. I handle these situations individually with compassion and flexibility.
7. Early Termination
If you choose to end a package engagement early, a prorated refund will be calculated based on appointments completed and any preparation or materials already provided. Early termination is handled individually at the time it occurs.
8. Proprietary Materials
Any resources, frameworks, templates, or materials I share with you as part of our work together are the intellectual property of Sage Consulting and Wellness LLC. They are provided for your personal use only and may not be reproduced, distributed, or shared without my written consent.
If I become aware that proprietary materials have been shared without consent, I reserve the right to review the situation and take appropriate action, up to and including early termination of the engagement.
9. Privacy and Confidentiality
What you share with me stays between us. I will not disclose your personal health information or the content of our work together to any third party without your consent, except as required by law.
Please note that wellness consulting services are not governed by HIPAA. However, I take your privacy seriously and handle all information with discretion and care.
10. Technology, Privacy, and AI-Generated Notes
Appointments are conducted virtually via Google Meet, part of Google Workspace. While Google Workspace provides strong privacy and security protections, please be aware that it is not a HIPAA-certified platform. By signing this agreement, you acknowledge and accept the use of Google Meet for our appointments.
To support the quality and continuity of our work together, appointments may be recorded for the sole purpose of generating an AI-assisted summary and notes. Recordings are stored securely in Google Drive and are not shared with third parties.
Following each appointment, I will review the AI-generated summary for accuracy and prepare a clear, easy-to-read version of your appointment notes. A copy will be emailed to you after each appointment for your records.
By signing this agreement, you consent to the recording of appointments for note-taking purposes. If you do not wish for an appointment to be recorded, please let me know before we begin and I will take notes manually instead.
11. Google Drive and Shared Documents
In addition to appointment notes, I use Google Drive to share resources, materials, and documents with you as part of our work together. You will receive access to a secure individual client folder that we can both contribute to. You are welcome to add documents, test results, provider notes, or anything else that supports our work together.
Please keep your login credentials secure and do not share folder access with others. Documents shared through Google Drive are intended for your personal use and should not be distributed without my consent.
12. Document Retention
Client records, notes, and shared documents will be retained for one year following your last appointment, after which they will be permanently deleted from Google Drive. Please save copies of any documents you wish to keep before that time.
13. Communication
You may reach me via email, phone, or text. I will respond within 48 hours during normal business hours, Monday through Friday, excluding holidays and weekends. If I will be unavailable for an extended period, I will notify you in advance so we can plan accordingly.
You are welcome to follow and interact with Sage Consulting on social media. However, direct messaging via social media is not a secure or appropriate channel for health-related communication. Please use email, phone, or text for anything related to our work together. Respectful interaction is expected across all platforms.
14. Distress and Referral Protocol
The scope of our work together is health and wellness consulting. If concerns arise during our work together that fall outside that scope, including significant psychological distress, I will discuss this with you and provide referrals to appropriate licensed providers.
This is not a crisis service. If you are experiencing a mental health emergency, please contact 988 (Suicide and Crisis Lifeline). For medical emergencies, please call 911.
15. My Responsibilities to You
As your wellness consultant, I am committed to:
- Coming to every appointment prepared, having reviewed your intake materials and any updates you have shared
- Providing information and guidance grounded in current research and best practices in women's midlife health
- Treating everything you share with care, discretion, and respect
- Being honest about the scope of my expertise and referring you to appropriate providers when your needs fall outside of what I can offer
- Responding to emails and messages within 48 hours during normal business hours
- Approaching our work together without judgment, meeting you where you are
- Maintaining clear professional boundaries that protect both of us and keep our work focused on your goals
16. Client Responsibilities
To get the most out of our work together, I ask that you:
- Complete intake forms prior to your first appointment
- Arrive on time and prepared for appointments
- Communicate openly about your goals, concerns, and any changes in your health
- Provide timely notice if you need to cancel or reschedule
17. Acknowledgment & Signature
By completing and submitting this form, you confirm that you have read and understood the Client Services Agreement above and that you agree to its terms.
